[Prevscilist] Important: Student Annual Evaluations Due June 5 at noon

Dear PREV students,

Hope you all are doing well and managing as best as possible during 
these challenging times. I continue to have office hrs on Fridays from 
12-1 and have had the chance to "see" some of you during that time. If 
you have any questions or just want to say hi, please feel free to drop 
by during my office hrs (on zoom -https://uoregon.zoom.us/j/268465201) 
or send me an email to schedule a different time.

I also want to share with you that the Annual Evaluations are due *June 
5 (Fri) at noon. *This email contains important information related to 
your annual evals. Please read carefully.

*Who needs to complete the annual evaluation? *All continuing (i.e, 
non-graduating) masters and doctoral students**need to complete the 
attached annual evaluation form in consultation with their advisor. 
Masters or doctoral students who are graduating this year (i.e., in 
Spring/Summer 2020) _DO NOT_ need to complete the annual evaluation.

*What do you need to do? *Students are supposed to fill their sections 
in the attached document and send it to their advisor for review and 
feedback. The advisor makes any necessary edits to the document, adds 
ratings, advisory summary, and any other feedback. The student schedules 
a meeting with the advisor to discuss the annual evaluation. Both 
student and advisor sign the document.

The annual eval form contains a section on advisor-advisee match - this 
section has been added to provide an opportunity for the student and the 
faculty advisor to discuss if the advising match/relationship is working 
well and identify any changes that need to be made. If you have any 
questions about this, you can contact your advisor, program director 
(atika at uoregon.edu), or associate program director (jcaouett at uoregon.edu).

*When is it due and to whom?* Student submits the signed document 
(electronic copy) to Danette Roberson (*prevsci at uoregon.edu*) by 
*Friday, June 5 at noon.
