[Prevscilist] help with Program Plan

Hi Danette,

How are you?

I am having trouble knowing which fields to put each of the research and 
elective classes in while completing this form. Would you have time to 
help me tomorrow morning? I have a bit of time from 9:30-10:00.  ( I 
will attach what I have done so far)

Also, does it have to be signed by our advisor prior to handing it in? 
Can it be an electronic copy of a signature?
