Dear Prospective Participant,

My name is Kiki Hachiya and I am a counseling psychology graduate student currently working on my dissertation project under the direction of Dr. Bianca Bernstein at Arizona State University. I would like to invite you to participate in my research study that explores the content of supervisee nondisclosure in supervision. You may participate if you are
1.      18 years or older
2.      A student of an APA accredited Counseling or Clinical Psychology doctoral program or a CACREP accredited Clinical Mental Health Counseling program, and
3.      Currently providing counseling through a practicum or internship experience and obtaining supervision of your clinical work.

Participation in this study should take about 15 minutes of your time to complete an online survey about your supervision experience. You can complete this survey on a computer, tablet, or mobile device. Your participation in this study is voluntary and all responses will be anonymous, as no identifiers will be collected. If you choose not to participate or to withdraw from the study at any time, there will be no penalty. There are no foreseeable risks or discomforts to your participation.

Your participation would be greatly appreciated. If you are interested in participating in this study, please click on the following link, which will direct you to the project information page and the survey. Upon completion of the questionnaire you will be given the option to enter a drawing to win one of eight $25 gift cards to Amazon. Participating in the drawing is optional and if you choose to participate, you will be directed to a separate survey that will not be connected to your responses, in any way. The e-mail you provide to be contacted with the drawing results will be kept confidential and separate from your survey responses. The email address you provide will be erased after the drawing.
