coe-staff: UO Face Mask Handout

Hello,

The UO is allowing us to do a group pick up for those who did not receive the free UO face masks when they were handed out earlier in the Fall.  If you did not already pick them up and are interested in getting them, please use the Qualtrics form below to put in your name, email address and 95# by close of business on Tuesday, December 8 (that's tomorrow).  Once we have the masks we will determine how to distribute them at the beginning of Winter term.
