cpsylist: externship agreement form

Dear CPSY students,

Attached is our updated externship agreement form. It will be posted on our blog very soon.
This form includes information that will be helpful to your externship supervisors, and you, regarding end of term evaluations and signing off on your hours.

If you are not completing an externship, the information below does not pertain to you right now.

If you have an existing externship agreement, you do not need to replace it with this one. However, please share this form with your supervisor
so they understand that Tk20 will be used to elicit the evaluation, and so they understand that from now on, the supervisor will report the
number of hours you have completed (direct intervention, assessment, supervision, other) within your evaluation.
You will have to provide that information to your supervisor for them to complete your evaluation.

If you are currently completing an externship and do not have an agreement in place, use this form.
To be clear, you should NEVER begin an externship without an agreement in place.
