<p class="MsoNormal">This form includes information that will be helpful to your externship supervisors, and you, regarding end of term evaluations and signing off on your hours.

<p class="MsoNormal">If you are not completing an externship, the information below does not pertain to you right now.<o:p></o:p></p>
<p class="MsoNormal"><o:p> </o:p></p>
<p class="MsoNormal">If you have an existing externship agreement, you do not need to replace it with this one. However, please share this form with your supervisor
<o:p></o:p></p>
<p class="MsoNormal">so they understand that Tk20 will be used to elicit the evaluation, and so they understand that from now on,
<u>the supervisor</u> will report the <o:p></o:p></p>
<p class="MsoNormal">number of hours you have completed (direct intervention, assessment, supervision, other) within your evaluation.
<o:p></o:p></p>
<p class="MsoNormal">You will have to provide that information to your supervisor for them to complete your evaluation.<o:p></o:p></p>

<p class="MsoNormal">If you are currently completing an externship and do not have an agreement in place, use this form.
