cpsylist: 2018 ANNUAL STUDENT EVALUATION- DUE May 10

Dear CPSY Students,



It's that time of year again!! Annual Evaluations!!



The current "2018 Annual Student Progress Report and Evaluation" is in response to several changes in the APA accreditation guidelines that went into effect January 1, 2017. Note that the evaluation contains a program progress/tracking section in which you will note completion of program courses and other milestones along 9 profession-wide competency areas that characterize APA accredited training programs in health service psychology.



Faculty will also complete some numerical ratings of your developing competencies in each of these areas, using a scale that is identical that that employed for evaluating your developing clinical training competencies in practica and externships.



Please fill in all dates, scores, information about completion of milestones, etc., and your advisor will fill in the scores. You will also complete the traditional, narrative self-evaluation components as you have done in the past, and your faculty advisor will provide a summary evaluation. You and your advisor will both sign once you have both reviewed the final document.



Please save your evaluation so that it is easier to complete next year!



By May 10 (sooner if possible) all students (including those on internship) should complete the attached annual student evaluation and provide an electronic copy of the completed form and all materials listed below.



Please FOLLOW DIRECTIONS CAREFULLY:



**  Complete the attached Evaluation template by adding requested content, dates, etc. into each section.



2.  Update your CV  (Use this format File name =  your last name.CV.5.18).



3.  Download your unofficial transcripts (can be obtained from DuckWeb. File name = last name.grades.5.18 ).



4.  Save each of these files using the specified file names provided.



5.  Then send all 3 items in a single email (with the 3 properly named attachments) to:

1.  your advisor and

2.  copy to Matt Marcott, our student services coordinator at (cpsy at uoregon.edu<mailto:cpsy at uoregon.edu>).



6.  Review points 1-5 to make sure all is in order.



If you have feedback or questions regarding the evaluation format (usefulness, clarity, questions, suggested improvements, etc.), please send that feedback in a separate email to Elizabeth at eskowron at uoregon.edu<mailto:eskowron at uoregon.edu> so she can address these and incorporate your ideas into the Electronic Version being developed in the COE.
