Governor's State Employees Food Drive 2021 - Payroll Deduction Instructions

Employees who wish to donate by payroll deduction will need to submit the form online using the link above. They will have the option to choose One-time Donation or spread their total donation over 12 months with the Monthly Deduction option. When you click the link, you will be directed through UO's Single Sign-On, which will trigger the Duo dual authentication service. This is necessary to access the form and sign up for payroll deduction.

Department: If you are combining teams with another department(s), be sure your employees know what to enter in this field.(E.g. Johnson Hall vs. VPFA)

Food Bank Selection:  FOOD for Lane County is the default food bank that will receive the donation. However, if an employee wishes to donate to another food bank in Oregon, they may access the drop-down menu and choose from that list.

One-time Donation:  Total amount will be deducted in the March 31 paycheck.
Monthly Deduction Donation:  Amount will be deducted in 12 equal donations over a 12-month period. For example, if $240 is entered in the Total Amount box, $20 per month will be deducted for 12 months, beginning with the March 31 paycheck.
Submitting Form:  Once the form has been completed, the employee must choose SAVE.

1.They will then be directed to submit the document.
2.You will need to click Sign Now. which is further down the page.  You will also receive an auto-generated email asking you to sign, even if you sign it at this point.  However, if you sign while on this page, no further action will be required.

Once this message is received, the form has been submitted correctly.
