coe-fac: Ordering Summer Regalia

Hello,
The Duck Store is revamping their regalia ordering website, so any orders should just be emailed to me.  If you will be participating in either the UO Summer Commencement or in Convocation in the fall and need to rent regalia, the information the Duck Store needs is below.  For summer commencement, your order needs to be in by July 17th.  When you reply, please also let me know which ceremony you are participating in.

Fill in the information below:
FACULTY MEMBER INFORMATION

NAME:       First and Last

SCHOOL:  Name of College or University

CITY:        The city where that College or University Campus is located.  The satin lining
                      colors of a hood can be different for each individual campus.  For example the
                      University of Michigan has two different hood satin combinations; Ann Arbor
                      and Dearborn are Maize and Royal Blue, Flint is Maize and Navy Blue.  If we
                      don't know the city we don't know which color of blue to provide.

COLORS:   If they know them, list them for us.  If not we can research them.

HEIGHT:    Feet and inches or meters.  Gowns are available in even-inch increments.

DEGREE:   Usually Ph.D. or Masters.  If the degree is a Masters or a Bachelors we need to
                       know the field of emphasis (History,  Biology, etc).

OTHER:    Anything special about the order.  A good example would be someone who
                      needs only part of the Regalia, for example only the gown or cap.
