BA News request - W-9/W-8 required for Vendor Address Changes effective 10/01/23

Effective 10/01/23 all vendor address changes will require a signed W-9 form submitted to Business Affairs Accounts Payable.  (Please note - a signed W-8 will be required for address changes to foreign vendors)

This change is part of the University's continuing efforts to avoid fraud and ensure the accuracy of our vendor records.

W-8/W-9 for address changes should be submitted securely via One Drive, in the same manner original vendor set-up documents are submitted to APBAO.  Instructions for secure submission can be found in the following link.

As of 10/01/23 any vendor address change submitted in any other format will be rejected and you will be asked to submit an updated W-8 or W-9 securely via One Drive.
