Flyer is in English and Spanish. It describes the free service to Oregon families and why it benefits parents of children ages 1 month to 72months.

Description:
No cost to use the site for parents to complete an Early Development Screening using the ASQ and/or ASQ:SE, parent-completed questionnaires. The site is confidential and anonymous, no names or accounts needed.

At-Home play activities for each age, community resources and guidance on connecting with an Oregon early intervention provider are included in a child's results letter.

Please let me know anytime if you have suggestions about where to post, others to send it to, or feedback on use or content of the site.
