The Center on Brain Injury Research and Training (CBIRT) at the University of Oregon is conducting a survey of the services and supports in Oregon for persons with brain injury.  We need your help!

The survey will inform future state and local policy changes necessary to better serve Oregonians who sustain brain injuries.

The survey takes about 30 minutes, but please complete at your own pace. The system will save your place if you need to take breaks while completing the survey.

Multiple individuals within a single organization or program can take the survey.  Please forward to other individuals, programs, and networks.

Both surveys are approved by the University of Oregon, Institutional Review Board. Accommodations are available. The survey can be completed in a paper version or with assistance by phone or video chat upon request.
