<p>Hello PREV students,</p>
<p>This email contains important information related to your annual
evals. Please read carefully. <br>
</p>
<p><b>Who needs to complete annual evals? </b>ALL PhD students, and
continuing/non-graduating masters students need to complete the
attached annual eval form in consultation with their advisor.
Masters students who are graduating this year (i.e., Spring/Summer
2019) <u>DO NOT</u> need to complete this form.</p>
<p><b>What you need to do? </b>Students are supposed to fill their
sections in the attached document and send it to their advisor for
review and feedback. The advisor makes any necessary edits to the
document, adds ratings, advisory summary, and any other feedback.
The student schedules meeting with the advisor to discuss the
annual eval and feedback. Both student and advisor sign the
document. <br>

<p><b>When is it due and to whom?</b> Student submits the signed
document (electronic or hard copy) to <b>Danette Roberson</b>
(PREV program coordinator) by <b>Friday, June 7 at 4 pm. </b><br>
</p>
