payroll: Updated Forms for International Employees (2015)

The Banner Guide has been updated to include the 2015 versions of international employee forms; individual forms and bundled hire packets.

The BAO website has been updated as well under the Payroll section for each employee class (Student, Faculty, & Staff).

You may start using the newly published forms for all hires. You may need to refresh your browser and/or delete cache to receive the latest version of the forms.
NOTE:  The Foreign National Data Request Form is no longer referred to as the 'CO-NRA' but is now the UO-NRA

Please recycle OLD forms and use the latest version of the forms:

  *   Foreign National Data Request Form(UO-NRA) updated to add calendar year 2015 under Part 3 and requests information for years prior to 2007 if applicable
  *   IRS Form 8233 changed to indicate 2015 calendar year on first page
  *   Attachment Letters for the Form 8233 have not been revised at this time

Note:  Any OLD versions of the forms submitted to the Payroll Office may delay processing the employee's pay.
The Annual Renewal emails will be sent to current non-resident alien employees beginning late next week instructing them to complete the newly revised 2015 UO-NRA and, if applicable, the Form 8233 and Attachment Letter.  You will be notified when this mailing has been sent out.
