This is just a friendly reminder that an External Practicum Agreement Form<https://community.uoregon.edu/courses/19063/files/257933?wrap=1> is required for any clinical activity that you do for which you intend to include on your internship application. This form is required at the start of the clinical activity. If you began a clinical activity this term that you want to include on your internship application, and you have not yet submitted an External Practicum Agreement Form<https://community.uoregon.edu/courses/19063/files/257933?wrap=1>, please submit this form completed by you, your advisor, and your site supervisor to cpsy at uoregon.edu<mailto:cpsy at uoregon.edu> ASAP and copy me (amariem at uoregon.edu<mailto:amariem at uoregon.edu>).
