Subject: Survey Distribution Request

Dear Dr. Tanner-Smith,

We hope this email finds you well. We are Counseling Psychology doctoral students at Saint Mary’s University of Minnesota, and are recruiting participants (i.e., students in health service psychology programs) for a new research study that we recently launched regarding the health promotion behaviors and training experiences of doctoral students in our field. Would you mind passing this along to students in your program? It takes approximately 15 minutes. Additional information, including IRB approval, can be found below. Thank you for your time and consideration!

Dear fellow doctoral student,

You are invited to participate in a research study that explores health promotion and training experiences of doctoral students in health service psychology. This study is being conducted by a team of students in the Counseling Psychology doctoral program at Saint Mary’s University of Minnesota. Our faculty sponsor is Dr. Erin Ayala.

You were selected as a possible participant in this study because you are a doctoral student in one of the health service psychology fields (Ph.D. or Psy.D. in Clinical Psychology, Counseling Psychology, or School Psychology).

There are no known risks or costs if you decide to participate in this research study. The information you provide will be used to better understand health promotion and training experiences of doctoral students in health service psychology programs. The survey will take about 15 minutes to complete. The information you provide may not benefit you directly, but will contribute to more general benefits by leading to increased knowledge in the field.

This survey is anonymous. This is a web-based survey, and while some demographic information will be asked for, this information will not identify you personally. No email or IP addresses will be collected. While we will do our best to provide anonymity, a guarantee of absolute anonymity cannot be guaranteed over the Internet. No one will be able to identify you or your answers, and no one will know whether or not you participated in the study. Should the data be published, no individual information will be disclosed.

Your participation in this study is voluntary. By completing and submitting the survey through the hyperlink provided, you are voluntarily agreeing to participate. You are free to decline to answer any particular question you do not wish to answer for any reason.

If you have any questions about the study, please contact Dr. Erin Ayala, Core Faculty, Psy.D. in Counseling Psychology, Saint Mary’s University of Minnesota, 2500 Park Avenue, Minneapolis, MN 55404, (612) 238-4515, eayala at smumn.edu<mailto:eayala at smumn.edu>.

If you would like to receive a summary of the results of the study, please include your email address at the end of the survey when prompted. Email addresses will be collected in a separate survey and thus will not be tied to the data you provide.

The Institutional Review Board for the School of Graduate and Professional Studies at Saint Mary’s University of Minnesota has reviewed and approved this project.
