New UO Substitute W9 and IRS Form W9

Good Morning,

We would like to notify you of a couple of recent changes to forms that are used to request vendor setup/maintenance.  To meet new State reporting requirements we have added 2 new categories to the UO Substitute W9 Form.  The new categories are Veteran Owned and Disabled Veteran Owned.  The Internal Revenue Service (IRS) has created a new version of their W-9 form.  The W-9 form is used by U.S. Citizens, Resident Aliens or U.S. entities.

In order to meet these State requirements, we ask Departments to obtain a UO substitute W-9 from vendors, if possible.  This will capture minority, women, veteran and emerging small business information that is not requested on the IRS W-9 form.  This information is used to collect annual spend information.  Following IRS regulations, we will not be able to accept prior versions of the 2018 W-9 forms after April 30, 2019.  The new version of the forms are available on Accounts Payable website https://ba.uoregon.edu/content/vendor-setup .
