Electronic version of the Mobile Technology Authorization Form

Good Afternoon,

We have developed an electronic version of the Mobile Technology Access and Payment Option Request (MTAPOR) form.  The form is designed to allow completion online and routing of approvals utilizing email, while retaining history of approvals and any changes that have taken place.  Here is a link to the form https://forms.uoregon.edu/form/launch/mobile-technology-access-and-payment-option-request.  If you get a chance to use the form please provide us feedback on any suggested changes or issues that you encounter.  Please keep in mind that a new MTAPOR form needs to be renewed annually if the stipend payment is to continue.
